What the Special Focus Facility designation means
The Centers for Medicare & Medicaid Services (CMS) uses its Special Focus Facility program for nursing homes with a history of serious quality problems. Its selection method considers the number, scope, and severity of deficiencies from health and complaint inspections. Selected facilities receive more frequent inspections and may face progressive enforcement. A candidate appears on a separate list; candidacy does not mean the home has been selected for the program.
Three New York facilities on the August 2026 active list
CMS’s report updated August 26, 2026 lists Van Duyn Center for Rehabilitation and Nursing in Syracuse (provider number 335184), Grand Manor Nursing & Rehabilitation Center in the Bronx (335744), and Waterview Heights Rehabilitation and Nursing Center in Rochester (335082) as current Special Focus Facilities. The report states that each facility’s most recent inspection did not meet the program’s survey criteria. This is a dated snapshot, and a later CMS posting may change the list.
The candidate list is longer
CMS separately lists New York facilities that are candidates for a future Special Focus Facility slot. Examples in the August report include Humboldt House Rehabilitation and Nursing Center in Buffalo, Livingston Hills Nursing and Rehabilitation Center in Livingston, and The Grand Rehabilitation and Nursing at Utica. Candidate status is different from active participation, and the candidate list is not a ranking of the state’s “worst” homes.
How to investigate a particular nursing home
Review the New York State nursing home profile and the facility’s inspection and complaint history. Ask about the dates, severity, subject, corrective action, and any later inspections. Compare the record with the resident’s actual care plan, nursing notes, staffing and treatment records. A quality designation or citation can identify a concern, but it does not alone establish that neglect caused a particular resident’s injury.
What families can do when care is unsafe
Seek prompt medical attention for an immediate health concern. Document observations and preserve dated photographs and communications. The New York State Department of Health accepts nursing home complaints. When a resident has suffered serious injury or death, an attorney can review the records and assess whether a specific lapse in care caused the harm.
